Palliative Chemotherapy with 5-Fluorouracil and Cisplatin for Advanced Head and Neck Cancer

Abstract
Background: The prognosis for patients with advanced head and neck cancer remains poor. Systemic chemotherapy is usually regarded as the standard treatment for palliation in patients with recurrent or metastatic cancer who have failed the definite local treatment with surgery and/or radiotherapy. The most active combination chemotherapeutic agents for the head and neck cancer are 5-fluorouracil and cisplatin (FP) regiemen.

Methods: 11 patients with distant metastatic or recurrent head and neck cancer were treated with 5-FU infusion and cisplatin between March, 1989 and May, 1994.

Results: 1) Among 10 evaluable patients, response rate was 50% (complete response 0%, partial response 50%). 2) All 5 Patients who achieved remission had progression of disease, eventually. Time to disease progression of each pa ient was 11, 27, 37, 44, 61 weeks. 3) The overall median survival of total 10 patients was 61 weeks. The median survival of the responders (median: not reached) to FP chemotherapy was not significantly prolonged compared with nonresponders (median: 26 weeks). 4) Leukopenia and thrombocytopenia were observed in 53% (grade I-III) and 9% (grade I) respectively. Nausea and vomiting were observed in all patients, but not severe. Alopecia, diarrhea, stomatitis, pneumonia were observed infrequently. There were no treatment related fatalities.

Conclusion: Combination chemotherapy with 5-FU and cisplatin in patients with distant metastatic or recurrent head and neck cancer seemed to be tolerable, but did not improve the response rate and overall survival compared with previous other reports. Randomized controlled prospective studies will be needed for the verification of this study.
